why on third cover, when printed, there is no 'episodes 6-7' below the 'Generation Kill'??!!

You can fix it in any graphic editor. Even in MS Paint or MS Word:
Text: "Episodes: 6-7"
Font: "Consolas"
Color: "Black"
Size: "10pt"
